Need Help?

  • Open the PDF in Adobe Acrobat Reader.

  • When you click the download button, the PDF may open in your browser. In this case, navigate to your "Downloads" folder in your "File Explorer" and open the PDF from this folder via Adobe Reader.

  • Fill out the left-hand side of the PDF.

  • Once completed, go to"File > Save As." Choose the folder you want to save the file in and then title the PDF with the player's name: "LastName_FirstName_2021TryoutForm."
